Smith of Kentucky; a son, Phillip W. Smith of North Vernon; a daughter, Chasidy Stevens of Columbus; brothers, Robert and Brad Smith both of Columbus, Rex Smith of Westchester, Ohio, and Allen Smith of Elizabethtown; and a grandson, Blake Stevens.
