Koffman, 51, of Central Avenue, died at 7:40 a.m. February 16, 2017, at Our Hospice Of South Central Indiana Inpatient Facility. The funeral will be at 1p.m. Monday, February 20, 2017, at Jewell-Rittman Family Funeral Home with Dr. William Bailey officiating. Calling hours will be from 1-5 p.m. Sunday at the funeral home and one hour prior to the service. Burial will be at Riverview Cemetery, Seymour. Memorial donations may be made to the Parkside Baptist Church or Muscular Dystrophy Association through the funeral home.
