Jay Barry Perkins, 54, of Hartsville, died at 11:06 a.m., October 5, 2021, at Our Hospice of South Central Indiana.
Jay was born October 21, 1966 in Columbus, the son of John and Helen Burton Perkins.
Survivors include sons, Luke (Brook) Perkins, of Columbus, Eli Perkins, of Hartsville and Cody Clouse, of Columbus; a daughter, Emily Perkins, of Hartsville; brothers, Mark (Yolanda) Perkins, of Memphis, TN., and John (Brenda) Perkins, of Newborn, Indiana; a sister, Sheila (Mike) Moore, of North Vernon; grandchildren, Kinsley and Macey Perkins, of Elizabethtown.
He was preceded in death by his parents, a brother, Matthew Perkins; a sister, Shelley Tichenor.
Services were entrusted to the Jewell-Rittman Family Funeral Home.

We had much fun together, tooling around in the convertible and just hanging out. We shared lots of laughter and you taught me a lot about nature. Your apples and tomatoes were delicious, along with your cherry jelly and homemade applesauce. Yum! I could go on and on, but I will keep your other memories close to my heart
